We are seeking a highly skilled Care Coordinator to join our team at NMC Healthcare. As a Care Coordinator, you will play a vital role in ensuring the smooth delivery of patient care services.

Key Responsibilities
  1. Communication and Resource Provision: Develop and maintain effective communication with patients, caregivers, and healthcare providers to ensure seamless coordination of care.
  2. Behavior Management: Recognize and manage challenging behaviors of patients and caregivers, providing support and resources as needed.
  3. Interdisciplinary Collaboration: Liaise with multidisciplinary teams to ensure comprehensive care planning and coordination.
  4. Data Management: Organize and manage patient information, ensuring accurate and timely data transfer to relevant stakeholders.
  5. Information Collection and Analysis: Receive, collate, and analyze information from various sources to inform care decisions.
  6. Risk Monitoring: Collaborate with healthcare providers to identify and monitor patients at risk, ensuring timely interventions.
  7. Quality Assurance: Conduct regular checks on data input to ensure quality and consistency.
  8. Reporting and Analytics: Generate reports to support population health management and inform care improvement initiatives.
  9. Meeting Coordination: Organize and manage minutes of internal meetings, ensuring effective communication and collaboration.

Requirements
  1. Education: Diploma or equivalent certificate in a health-related field, or a Bachelor's degree in a related field.
  2. Experience: Minimum 2 years of experience in a healthcare setting, with a focus on administrative duties and database management.
  3. Skills: Excellent verbal and written communication skills, with proficiency in medical terminology and common software applications.
  4. Language: Fluency in English and Arabic, with strong organizational and documentation skills.
  5. Critical Thinking and Problem-Solving: Strong critical thinking and problem-solving skills, with the ability to analyze complex data and inform care decisions.
