We are seeking a highly skilled Clinical Case Manager to join our team at Actalent. As a key member of our healthcare services team, you will play a critical role in delivering high-quality care to our clients.

Key Responsibilities
  • Develop and Implement Care Plans: Determine, develop, and implement comprehensive care plans based on accurate and thorough assessments of clients' needs.
  • Collaborate with Healthcare Professionals: Work closely with clinical nurses, treating physicians, and other healthcare professionals to ensure seamless care coordination.
  • Apply Clinical Knowledge and Guidelines: Utilize detailed knowledge of established medical policies, clinical practice guidelines, and community resources to inform care decisions.
  • Provide Guidance and Support: Offer guidance and support to healthcare providers through telephone triage and effective discharge planning.
  • Manage Complex Cases: Work with complex cases, promoting the delivery of quality, cost-effective healthcare services based on medical necessity and contractual benefits.
Requirements
  • Education and Experience: Bachelor's of Science in Nursing or advanced degree preferred. Active Oklahoma RN license required. 5 years of experience in case management, utilization review.
  • Specialized Training: Must have specialty training in one of the following specialized areas: case management, utilization review.
