Care Coordinator

We are seeking a highly skilled and compassionate Care Coordinator to join our team at ClickJobs. As a key member of our care team, you will play a vital role in providing exceptional support to our clients and their families.
Responsibilities- Provide care coordination services to clients with mental health and substance use issues, including assessment, planning, and linkage to community resources.
- Collaborate with a multidisciplinary team to develop and implement individualized care plans that address the unique needs of each client.
- Provide education and support to clients and their families on mental health and substance use issues, as well as connect them with community resources and services.
- Manage a caseload of clients and their diverse needs, ensuring timely and effective interventions.
- Develop and maintain relationships with community partners and providers to ensure seamless care coordination and referrals.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures that promote best practices in care coordination.
- Bachelor's Degree in Psychology, Social Work, Counseling, or a related field, or one year of experience in the behavioral health field for non-related Bachelor's degrees.
- Experience working with children, teens, and adults in an outpatient setting, with a strong interest in working with adults with mental illness and substance use issues.
- Experience in provision of care coordination, teaching skills, and supportive counseling.
- Collaborative and client-centered orientation, with excellent communication and interpersonal skills.
- Ability to manage a caseload of clients and their diverse needs, with a strong commitment to delivering high-quality care.
- Spanish and English speaking required.
