Job Title: Care Coordinator

At ClickJobs, we are seeking a highly skilled and compassionate Care Coordinator to join our team. As a Care Coordinator, you will play a vital role in ensuring the seamless delivery of healthcare services to our clients.

Key Responsibilities:
  • Engage and Retain Clients: Develop and maintain strong relationships with clients, their families, and caregivers to ensure their needs are met and they receive the necessary support.
  • Coordinate Care Services: Collaborate with healthcare providers, specialists, and community organizations to coordinate and arrange for the provision of services, including medical, behavioral health, and social services.
  • Develop Individualized Care Plans: Work with clients and their families to create comprehensive, person-centered care plans that address their unique needs and goals.
  • Ensure Timely Transitions: Coordinate transitions between healthcare settings, including hospitals, rehabilitation centers, and home care, to ensure smooth and safe care transitions.
  • Monitor and Evaluate Client Needs: Regularly assess client needs and make adjustments to care plans as necessary to ensure optimal outcomes.
  • Communicate Effectively: Maintain accurate and timely documentation, and communicate effectively with clients, families, and healthcare providers to ensure seamless care coordination.
  • Collaborate with Healthcare Teams: Work collaboratively with healthcare teams, including physicians, nurses, and social workers, to ensure comprehensive and coordinated care.
  • Stay Up-to-Date with Industry Developments: Participate in ongoing training and professional development to stay current with industry best practices and advancements.
Requirements:
  • Education: Bachelor's degree in social work, psychology, or a related field.
  • Experience: 2+ years of experience working with vulnerable populations, preferably in a healthcare setting.
  • Skills: Excellent communication and interpersonal skills, ability to work independently and as part of a team, and proficiency in electronic health records and other software applications.
  • Language: Fluency in English, with the ability to communicate effectively with clients and families from diverse backgrounds.
