Key Responsibilities:

  • Conduct comprehensive psychosocial assessments of patients and their families to identify needs, strengths, and resources.
  • Develop individualized care plans and goals in collaboration with patients, families, and healthcare teams.
  • Provide counseling and emotional support to individuals and groups facing social and emotional challenges.
  • Advocate for patients' rights and assist them in accessing community resources, social services, and government assistance programs.

  • Excellent communication, advocacy, and crisis intervention skills.
  • Ability to work effectively both independently and as part of a multidisciplinary team.
