EssentialResponsibilities

  • Prepares and evaluates assessments,development and implementation of social work service plans.Provides psychosocial assessment, diagnostic impressions, treatmentof individuals/couples/families/groups, prevention of psychosocialdysfunction/disability/impairment includingemotional/mental/behavioral disorders, and evaluation of practiceeffectiveness. Applies knowledge base, theoretical concepts,specific functional skills, and essential social values used toeffect change in human behavior, emotional responses, and socialconditions. Helps enhance or restore personal and socialfunctioning.
  • Performs case management,coordination, casework intervention, and monitoring of social workservice plans in areas of personal, social, or economic resources,conditions, or problems. Provides information, resourceidentification and referral services, mediation services, advocacyservices, and education of individuals, groups, couples, andfamilies. Coordinates services and assures patients placement in anappropriate setting.
  • Assists withadministration, development, implementation, and evaluation ofsocial work programs and policies. Conducts social workconsultation and resource development. Plans for and participatesin development activities to keep abreast of current trends inmedical social work. Participates in performance improvement.Implements Quality Improvement processes. Assists with developmentand implementation of departments annual work-plan. Performsclinical record audits. Maintains educational record and providesevidence of competency.
  • Conducts researchthrough formal design/methodology of data collection andanalysis/evaluation of data, social work programs, social systems,and social policies. Documents according to departmental standards,Federal and State regulations. Assures medical recorddocumentation.
  • Participates in departmentalfunctions such as staff meetings, staff in-services, andcommittees, as assigned. Participates in orientation of newpersonnel.

  • Masters degree in socialwork.

License,Certification, Registration

  • Licensed Social Worker (Hawaii) ORLicensed Clinical Social Worker (Hawaii)
  • BasicLife Support required at hire from American HeartAssociation

AdditionalRequirements

  • Healthcare experience working withmedical diagnoses.
  • Demonstrated ability innetworking with community agencies.
