**Key Responsibilities**:
1. Conduct comprehensive assessments of clients’ social, emotional, and economic needs, including interviews, observations, and evaluations of client circumstances and resources.

2. Develop individualized care plans and goals in collaboration with clients and their families, based on assessment findings, strengths, and needs.

3. Provide counseling, support, and guidance to clients and families facing social, emotional, or behavioral challenges, and assist them in developing coping strategies and problem-solving skills.

4. Advocate for clients’ rights and interests, and assist them in accessing and navigating social services, government assistance programs, and community resources.

5. Coordinate and facilitate referrals to other professionals, agencies, and organizations for additional services and support, such as mental health counseling, housing assistance, or financial aid.

6. Monitor and evaluate clients’ progress towards goals, and adjust care plans and interventions as needed to address changing needs and circumstances.

**Requirements**:
1. Master’s degree in Social Work (MSW) from an accredited program; state licensure or certification as a Licensed Clinical Social Worker (LCSW) is preferred.
2. Previous experience in social work, counseling, or related fields, preferably in a healthcare, social services, or community-based setting.
3. Strong understanding of social work principles, theories, and interventions, with knowledge of human behavior, psychology, and social systems.
4. Excellent communication and interpersonal skills, with the ability to establish rapport and build trust with clients from diverse backgrounds.
5. Empathy, compassion, and cultural competence, with the ability to work effectively with individuals and families facing social and economic challenges.
