Position Summary
A Medical Records clerk is responsible for performing the daily tasks of the Medical Records department as agreed by the Medical Records Supervisor. The Medical Records clerk will assume role of Senior Medical Records Clerk in his/her absence
Summary of Main Duties
- Actively participates in upholding and promoting the vision and culture of University Hospital Sharjah (UHS)
- Complies with all UHS policies and procedures
- Participates in all aspects of the International Accreditation program to assist UHS attain and maintain international accreditation standards
- Performs activities of a medical records department that includes the planning and organizing of the documentation, storage and retrieval of medical records
- Monitoring & Evaluation of the department performance, reporting any significant findings back to the Medical Records Supervisor
- Assists the Medical Records Supervisor with the promoting of the development of the Medical Records department, through scan at source projects, elimination of paper records through conversion to electronic
- Implements data collection as required using agreed coding methods
- Participates in the development and implementation of policies to preserve the confidentiality of information contained in the medical records and monitors compliance
- Assists to resolve any problems that arise with releases of confidential medical information
- Assists with the monitoring of the accurateness of patient records and documentation of patient visit
- Maintains a referral tracking system to insure proper follow-up of patient care referred for outside consultations and imaging services
- Orientates new staff to the relationship and interaction of medical records unit with other units of the facility
- Participates in applicable committees
- Emailing the doctors with deficiencies of the Medical record
- Auditing the diagnosis on the Trakcare system on daily basis
- Releases medical information on client's and clinician's request and requests medical information from other medical facilities
- Responsible in scanning registration form, general consent form, surgical consent form, general anesthesia consent form, epidural consent form, blood transfusion consent form, CTG, ECG, endoscopy report and film, hearing test, and outside laboratory results and medical reports, then attaching them on Trakcare System
- Maintain numeric filing system by organizing patient records on shelves to ensure records are readily accessible by all departments
- Keeps supervisor informed of problems or issues, monitors supplies needed, and performs other duties as assigned
- Preparing patients’ file that is scheduled in the OT for the following day
- Receiving patients’ files from different wards mainly OB/Gyne, Pediatric, Surgical, and OR that need to be fixed and scan important documents that needed to be attach on the Trakcare system
- Creates folders for new patients, prepares file labels, and putting code tabs
- Responsible in inserting documents in patients’ files and arranging them in a chronological manner
- Locates, signs out, route and delivers requested medical records to the appropriate personnel and departments
- Maintains numeric filing system by organizing patient records on shelves to ensure records are readily accessible by all departments
- Bachelor’s Degree Graduate
- Minimum of 2 years Medical Records experience at clerk level
Position criteria Other Skills/Abilities
- Proficient in English, written and verbal, with Arabic preferred
- Previous exposure to healthcare industry preferred
- Computer knowledge
- Typing speed of at least 40wpm
