**Key Responsibilities**:

- **Resident Health Management**:

- Maintain and restore the health of resident patients through high-quality care.
- Act as the primary contact between residents and their families, addressing and preventing concerns.
- **Patient Care Planning**:

- Assist in the development, implementation, and adjustment of patient care plans.
- Collaborate with Nurse Practitioners and Physicians to review and update care plans regularly.
- **Staff Supervision and Development**:

- Supervise, counsel, and discipline nursing staff to ensure quality care.
- Ensure staff adhere to company policies, federal, and state regulatory standards.
- **Compliance and Quality Assurance**:

- Ensure unit meets all regulatory requirements and company policies.
- Conduct audits on care plans, charting, medical forms, and other administrative records to uphold standards.
- **Hands-on Care and Administrative Support**:

- Deliver hands-on care and distribute medicine as needed.
- Complete admissions, discharge assessments, and oversee chart evaluations and updates.
- **Problem Identification and Resolution**:

- Identify procedural issues and work with the Nursing Supervisor to implement solutions.
- Prioritize and address the unique care needs of each resident, ensuring a responsive and tailored approach to health management.

**Requirements**:

- Proven experience in healthcare operations, preferably in a homecare or residential care setting.
- Strong interpersonal and communication skills to liaise effectively with patients, families, and medical staff.
- Demonstrated ability to manage and lead a team in a fast-paced environment.
- Knowledge of healthcare regulatory standards and procedures.
- Ability to conduct audits and manage administrative tasks related to patient care.
