Responsibilities:

- Provide ongoing case management services, including regular checkins and followup
- Monitor progress toward goals and adjust the service plan as needed

- Advocate for clients and connect them with appropriate resources and services
- Maintain accurate and uptodate case files and documentation
- Provide crisis intervention and support as needed
- Participate in team meetings and case conferences
- Keep uptodate with industry standards and best practices for case management
Requirements:
- Bachelor's degree in social work, psychology, or related field
- 4+ years of experience in case management or related field
- Knowledge of social services and community resources
- Strong interpersonal and communication skills
- Ability to work independently and as part of a team
- Excellent organizational and time management skills
- Strong problemsolving and criticalthinking skills
- Ability to maintain confidentiality and professional boundaries
- Valid driver's license and reliable transportation
