Key Responsibilities
  • Assessment and Charting: Conduct thorough assessments of patients' conditions at each visit, documenting observations and charting progress.
  • Medication and Vital Signs Review: Complete evaluation tasks, including reviewing medication and vital signs to ensure patients receive optimal care.
  • Wound Care and Education: Dress or redress wounds, assess healing progress, and provide education to patients and families on proper home health care procedures and strategies.
  • Interdisciplinary Collaboration: Coordinate with Physicians, Physical Therapists, Occupational Therapists, and other healthcare professionals to develop and implement comprehensive care plans.
  • Palliative Care: Provide comfort and support to patients and their families through palliative care, focusing on pain management and symptom relief.
  • Equipment Recommendations: Identify and recommend tools or devices that can improve patients' quality of life.
Requirements
  • Experience: 1+ years of experience as a Home Health Nurse.
  • Soft Skills: Active listening skills, empathy, physical and mental strength, initiative, ability to act swiftly in emergencies, flexibility, and patience.
  • Education: Bachelor's degree in Nursing.
