**Key Responsibilities:**

  • **Clinical Tasks:**
    • Prepare and organize patients' office visits, ensuring a smooth and efficient experience.
    • Perform focused nursing assessments to identify health maintenance gaps and implement planned nursing interventions.
    • Recognize when to defer to RN/clinician and utilize standardized tools to identify health maintenance gaps.
    • Contribute to individualized patient care plans and prepare patients for visits/procedures.
    • Ensure exam/procedure rooms are ready and provide health education to patients.
    • Document nursing care in KP HealthConnect and update patient records.
  • **Clinical Procedures:**
    • Perform diagnostic tests, therapies, and procedures within the scope of practice competently and with maintenance of proficiency.
    • Examples include screening tests (vital signs, vision, hearing), diagnostic tests (urine dips, spirometry), medication administration, wound care, therapies, tube insertions, minor surgical procedures, lab specimen collection, irrigations, and instrument/equipment setup.
  • **Practice Management Activities:**
    • Maintain clinician schedules and respond to patient communications in a timely and professional manner.
    • Facilitate follow-up duties on patient care plans and assist in coordinating patient care.
    • Sort mail containing clinical information and maintain accurate and up-to-date patient records.

**Requirements:**

  • Current LPN licensure and certification in wound care.
  • Strong clinical skills and ability to work effectively in a fast-paced environment.
  • Excellent communication and interpersonal skills.
  • Ability to maintain confidentiality and handle sensitive patient information.
