The Case Coordinator works closely with a team of professionals and stakeholders to ensure clients receive the support and services they need.
**Key Responsibilities**:
- Provide patients with coordinating the services, including intake team coordination, assessment, planning, and document collection.
- Coordinate and collaborate with stakeholders, including clients, colleagues, and community partners, to ensure that clients receive comprehensive services and support.
- Develop and maintain patient records using case notes, progress reports, and other documentation.
- Monitor patient progress and ensure timelines and goals are met, making adjustments as needed.
- Participate in team meetings and case reviews to ensure patients receive coordinated and effective services.
- Maintain up-to-date knowledge of relevant policies, procedures, and regulations related to case management and patient services.
- Maintain patient confidentiality-related information and adhere to ethical and professional standards.
**Qualifications**
- A bachelor's degree in a relevant field such as social work, psychology, nursing, or healthcare administration
- With 2-3 years of experience in the same field
- Should have excellent communication and interpersonal skills and strong organizational and time management skills.
- Should be able to work under pressure, adapt to changing circumstances, and maintain confidentiality and professionalism at all times.
- Can start immediately.
- Willing to be assigned/relocate to Abu Dhabi, UAE
