Establishes plan of care to achieveestablished goals relative to age, medical history anddysfunction.
Administers or prescribesmodalities and other physicalagents.
Administers or prescribes exerciseprograms/development of patient careprograms.
Demonstrates professional competenceand clinical reasoning in assessing patients condition at the onsetand through all phases of skilled physicaltherapy.
Routinely takes usual and specialprecautions relative to the age, medical history and condition ofthe patient, and the type of treatment beinggiven.
Regularly reassesses clinical signs andsymptoms to determine effectiveness of treatment, progressiontowards goals, and the need for modification of treatment and/orgoals.
Implements the treatment programthrough direct treatment of the patient.
Maydelegate a portion of the treatment plan to appropriate supportivepersonnel.

Follows department guidelines oncleanliness of assigned areas and reports non-functioning ofpatient care equipment by following department equipmentmaintenance policies and procedures.
Providesaccurate and timely documentation of physical therapy servicesprovided per State and Federal requirements and relevant medicalgroup policies and guidelines.
Works inpartnership with Management to complete all mandatory compliancerequirements.
May be required to be trained tocover a diverse population of patients depending on local medicalcenter need.
May be involved in the clinicalcompetency of PT Assistants and PTAides.
Demonstrates internal and externalcustomer service at all times and behaves in accordance withPrinciples of Responsibility.
In cooperationwith management, acts as a resource for department issues in theimmediate area regarding day to day operations such as providingoperational direction to staff PTs, PTAs, and PT aides i.e. assistswith orientation and training of employees, informing management ofany barriers preventing successful completion of training; assistsin the completion of new employee core competencies, providetechnical direction and support to employees, inform management onthe work environment (e.g., staffing, training/competencies, andequipment needs).
Works in cooperation withmanager to ensure proper safety, maintenance, and levels ofequipment and supplies.
Provides input toclinical performance of staff around routine procedures asappropriate.
Provides input to managementregarding the performance evaluation of assignedstaff.
Works in partnership with managementand is actively involved with staffing models and smooth staffingacross areas.

Experience
BasicQualifications:
Four (4) YearsMinimum Physical Therapist ExperienceRequired.
Education
Completionof an accredited PT training program with a Bachelors, Masters, orDoctorate degree in Physical Therapy.
License,Certification, Registration
Physical TherapistLicense (California) required at hire OR Physical Therapist License- Applicant (California) required athire
Basic Life Support required athire
National Provider Identifier required athire
AdditionalRequirements
Must be able towork in a Labor/Management Partnershipenvironment.
