This includes assessing patient needs, developing care plans, administering medications, and educating patients and their families on managing their conditions. You will collaborate with other healthcare professionals to ensure comprehensive and holistic care for patients.Requirements:Valid DOH Registered Nurse licenseMinimum of 2years of experience in homecareStrong clinical skills and knowledge of nursing principlesExcellent communication and interpersonal skillsAbility to work independently and as part of a multidisciplinary teamCompassionate and patient-centered approach to carevalid BLS , ACLS requiredResponsibilities:Conduct initial assessments and ongoing evaluations of patient needsDevelop and implement individualized care plans in collaboration with the patient, their family, and other healthcare providersProvide skilled nursing care, including wound care, medication management, and infusion therapyMonitor patient vital signs and symptoms, and report any changes or concerns to the healthcare teamEducate patients and their families on disease management, medication administration, and other aspects of careDocument all patient interactions, assessments, and interventions accurately and in a timely mannerCoordinate care with other healthcare providers and community resources as neededBenefits:Competitive salaryHealth insurancePaid Annual LeaveSupportive work environment
