Navigating health and aging is hard. Baba makes it easier by pairing each patient with a real human advocate team that supports them between visits. Baba helps seniors and caregivers manage appointments, billing questions, and complex healthcare decisions, supporting thousands of families across assisted living, nursing homes, and at-home settings.
ResponsibilitiesDevelop trusting relationships with patients through listening, respect, and clarityAssess medical, social, and environmental factors to pinpoint barriers and needsCreate individualized care plans integrating medical, social, and behavioral supportsConnect patients to local services such as transportation, meal delivery, and home careAnalyze medical bills and assist with claim appeals or explanationsIdentify medication cost savings, co-pay assistance, or formulary optionsLink patients to social support networks and community programsOrganize and maintain medical records, care histories, and provider communicationCoach patients on best practices for managing conditions and navigating careDocument interactions and patient progress in Baba's systemsCollaborate with clinical, product, and operations teams to optimize workflowsRequirementsBachelor's degree in Nursing, Social Work, Public Health, Health Administration, or related field2+ years of experience in care management, patient advocacy, case management, or clinical social workExperience supporting older adults or Medicare populationsSolid knowledge of Medicare rules, eligibility, benefit structures, claims, and appealsDeep understanding of Social Determinants of Health (SDOH)Proficiency with digital tools, EMRs, and care-management softwareMinimum 20 hours of availability per week, including 75% during standard business hoursPreferred QualificationsActive RN, LPN/LVN, LSW/LMSW/LCSW, CCM/ACM, or CHW credential
