EssentialResponsibilities

  • Plans, develops, assesses andevaluates care provided to members. Collaborates with physicians,other members of the multidisciplinary health care team andpatient/family in the development, implementation and documentationof appropriate, individualized plans of care to ensure continuity,quality and appropriate resource use. Reviews, monitors, evaluatesand coordinates the patients hospital stay to assure that allappropriate and essential services are delivered timely andefficiently. Communicates via huddles with hospitalist partnermultiple times throughout the day.
  • Reviews allnew inpatient admissions within 24 hours and begins the dischargeplanning process immediately. Assesses high risk patients in needof post-hospital care planning. Develops and coordinates theimplementation of a discharge plan to meet each patients identifiedneeds; communicates the plan to physicians, patient,family/caregivers, staff and appropriate community agencies toenhance the effect of a seamless transition from one level of careto another across the continuum. Ensures that the appropriate levelof care is being delivered in the most appropriate setting.Recommends alternative levels of care and ensures compliance withfederal, state and local requirements.
  • Performspsychosocial assessments on all patients that meet the high riskindicators for discharge planning. Comprehensively assessespatients goals as well as their biophysical, psychosocial,environmental, economic/financial, and discharge planning needs.Provides patients with education to assist with their discharge andhelp them cope with psychological problems related to acute andchronic illness. Refers patients to the ambulatory case managers,care managers and/or social workers as appropriate. Documents alladmissions and discharges in the patients Kaiser Permanenteelectronic medical record. Makes post discharge follow-up calls toall patients who are not referred to an ambulatory case/caremanagement program.
  • Attends scheduled rounds 2times/week with the Physician Director of Resource Stewardship todiscuss clinical course and discharge planning for assignedpatients identifying any real or potential delays in care orquality of care issues.
  • Acts as a liaisonbetween inpatient facility and referral facilities/agencies andprovides case management to patients referred, serving as anadvocate for patients and families. Coordinates transfer ofpatients to appropriate facilities; maintains and provides requireddocumentation. Builds highly effective working relationships withphysicians, SNF staff, vendors, and other departments within thehealth plan.

Experience

BasicQualifications:

  • Minimum two (2) years of experiencein patient care delivery or completion of Masters degree in CaseManagement Program in lieu of minimum years ofexperience.

Education

  • Associates Degree Nursing.

License, Certification,Registration

  • RegisteredProfessional Nurse License (Georgia)
  • LicensedRegistered Nurse (Temporary - Georgia)

AdditionalRequirements

  • Demonstrated advanced communicationand interpersonal skills with all levels of internal &external customers, including but not limited to medical staff,patients and families, clinical personnel, support and technicalstaff, outside agencies, and members of thecommunity.
  • Ability to collaborate effectivelywith multidisciplinary healthcareteam.
  • Excellent time management skills with theability to work successfully in a fast-paced environment. Must beself-directed, and have the ability to tolerate frequentinterruptions and a demanding workload.
  • Functional knowledge ofcomputers.
  • Experience with managed health caredelivery including Medicare.
  • Experience in apayer environment highly desirable.
  • Knowledgeof funding, resources, services, clinical standards, and outcomesis preferred.
  • Complex Case ManagementCertification preferred.
  • Knowledge of the NursePractice Act, TJC, DMHC, CMS, NCQA, HIPPA, ERISA, EMTALA &all other applicable federal/state/local laws ulations.
  • Demonstrated strong communicationand customer service skills, problem-solving, critical thinking,& clinical judgmentabilities.
  • Fundamental word processing& computer navigation skills & the ability tointerpret & use analytic data in day to dayoperations.
  • Knowledge of healthcare benefitsassociated with various business lines.

PreferredQualifications

  • Minimum five (5) years of clinicalnursing experience in a hospitalsetting.
  • Minimum five (5) years of professionalpractice experience in an acute caresetting.
  • Minimum two (2) years of experience inutilization review, case management, and discharge planningpreferred.
  • Bachelors Degree in Nursing, HealthCare or Masters degree
