Key Responsibilities
  • Home Visiting Services: Provide regular home visits to pregnant and parenting individuals and their families, focusing on education, case management, and support.
  • Case Management: Develop and implement individualized plans to address the physical, emotional, social, and environmental needs of clients.
  • Education and Support: Provide education and support to clients on topics such as parenting, child development, and health promotion.
  • Collaboration and Consultation: Work closely with other healthcare professionals, community agencies, and social services to ensure comprehensive care and support for clients.
  • Documentation and Reporting: Maintain accurate and timely documentation of client interactions, progress, and outcomes.
Requirements
  • Education: Bachelor of Science in Nursing (BSN) or Associate Degree in Nursing (ADN) with current enrollment in a BSN program.
  • Licensure: Current New York State Registered Nurse (RN) license in good standing.
  • Experience: Two years of recent experience in maternal and child health, public health, home visiting, or mental/behavioral nursing.
  • Skills: Excellent written and verbal communication skills, basic computer skills, and ability to work a flexible schedule including evenings and weekends.
Preferred Qualifications
  • Language Skills: Bilingual English/Spanish required.
  • Home Visiting Experience: Previous experience in home visiting preferred.
