We are seeking a highly motivated and experienced Nurse Coordinator to join our team at Axios International Consultants. As a Nurse Coordinator, you will be responsible for providing comprehensive nursing care to patients in the community setting, working closely with physicians and other healthcare professionals to ensure the best possible outcomes.
About the Position- The successful candidate will have a strong background in community nursing or home care, with experience in providing skilled nursing services to individuals and/or families in the community.
- You will be responsible for telephonic follow-up consultation, monitoring of vital signs, administering medications, and pain management medication monitoring.
- Additionally, you will be required to educate and counsel patients and their families on various health topics, conduct wound assessments and dressing changes, and supervise health teaching and support for family and caregivers.
- The Nurse Coordinator will also be responsible for attending and completing pharmacovigilance, pharma, and product training provided by the client, and reporting any adverse events/product complaints as per client's requirements.
- Maintaining accurate records of patient care and progress.
- Conducting regular assessments of patients' physical and mental health.
- Providing education and support to patients and their families on various health topics.
