As a Clinical Case Manager, you will be responsible for promoting quality and cost-effective interventions and outcomes. You will work closely with various departments to develop collaborative strategies and ensure seamless patient care.
Responsibilities
- Promote quality and cost-effective interventions and outcomes by coordinating with cross-functional teams.
- Record cases information accurately, complete necessary forms, and produce statistical reports to support data-driven decision-making.
- Foster effective working relationships with administrative, medical, and RCM teams throughout the case management process.
- Handle case assignments, draft service plans, review case progress, and determine case closures to ensure timely resolution.
- Evaluate healthcare treatments and negotiate cost-effective options to manage DRG effectively.
- Assist in timely discharge planning to optimize patient outcomes.
- Evaluate documentation rigor and code assignment to ensure fair remuneration.
- Develop training sections to prevent rejections and improve overall quality.
- Audit files to identify vulnerabilities and alert concerned departments.
- Perform all other duties assigned by hospital administrators while maintaining compliance with OSH policies and procedures.
