Position Overview:
This role involves providing essential home-based healthcare services to expectant and new parents and their families, ensuring they receive the support they need through the NFP Program.

Key Responsibilities:

  • Conducts home visits for eligible pregnant and parenting individuals and their families.
  • Delivers education and case management services primarily in the Far Rockaway area, supporting families from pregnancy through the child's second birthday.
  • Follows the nursing process and adheres to the NFP model of home visitation.
  • Maintains a caseload of at least 25 clients.

Clinical Responsibilities:

  • Establishes therapeutic relationships and employs reflection and motivational interviewing techniques during home visits.
  • Conducts home visits in alignment with NFP guidelines and best practices.
  • Implements the nursing process while integrating NFP into nursing care.
  • Evaluates the physical, emotional, social, and environmental needs of families as they relate to NFP domains.
  • Assists families in setting and achieving their health goals.
  • Provides educational resources, support, and referrals to help families meet their objectives.
  • Collaborates with other professionals to ensure comprehensive service delivery.
  • Develops nursing diagnoses based on assessments and client goals.
  • Facilitates referrals for necessary services and follows up to confirm families are receiving appropriate care.
  • Completes all required documentation for billing purposes.
  • Performs additional duties as necessary.

Qualifications:

  • Bachelor of Science in Nursing (BSN) required, or an associate degree if currently enrolled in a BSN program.
  • Active New York State RN license in good standing is mandatory.
  • Current CPR certification is required.
  • Bilingual proficiency in English and Spanish is essential.
  • Preferred: Two years of recent experience in maternal/child health, public health, home visiting, or mental/behavioral nursing.
  • Home visiting experience is a plus.
  • Strong written and verbal communication skills are necessary.
  • Basic computer skills are required.
  • A valid driver's license and insured vehicle may be necessary.
  • Flexibility to work evenings and weekends is expected.

Compensation:
Salary: $85,000.00
