Key Responsibilities
  • Assess and Evaluate Patient Needs: Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and developing targeted care plans to address these needs.
  • Develop and Implement Care Plans: Collaborate with patients, families, and healthcare providers to develop and implement comprehensive care plans that meet the unique needs of each patient.
  • Provide Skilled Nursing Care: Provide skilled nursing care, including wound care, medication management, and other interventions as needed, to patients in their homes.
  • Communicate Effectively: Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination and to address any concerns or questions that may arise.
  • Document Patient Information: Accurately and thoroughly document patient information, including assessment data, care plans, and progress notes, in a timely and efficient manner.
Requirements
  • Education: Associate's degree in Nursing from an accredited program.
  • Experience: Minimum two (2) years of experience as a Registered Nurse in medical-surgical, home health, or hospice.
  • Licensure: Current Registered Nurse License (Washington) required at hire.
  • Skills: Basic laptop skills, including MS Office Suite, and customer service skills.
Preferred Qualifications
  • Home Health Care Experience: Demonstrated experience providing care planning within a community setting (continuum of care).
  • Horizon for Home Care System Experience: Experience with the Horizon for Home Care system.
