Responsibilities

  • Care Coordination: You will work closely with our healthcare team to develop and implement effective care coordination plans that meet the unique needs of each patient.
  • Utilization Reviews: You will conduct thorough utilization reviews for patients who have been admitted to our facilities, ensuring that they receive the necessary care and services to meet their needs.
  • Clinical Reviews: You will conduct thorough clinical reviews based on established treatment criteria, ensuring that patients receive the necessary care and services to meet their needs.
  • Interviews and Assessments: You will interview patients and caregivers regarding care after hospitalization and assess their needs to ensure a smooth transition to post-acute care.
  • Quality Improvement: You will generate quality improvement results and report unusual occurrences or inappropriate decisions to your supervisor.

Requirements

  • Experience: A minimum of two years of experience in an inpatient setting or skilled nursing facility, with a minimum of one year of utilization experience required.
  • Education: A bachelors degree in nursing or a related field is required, or four years of experience in a directly related field.
  • Licenses and Certifications: A valid California driver's license, Registered Nurse License (California), and Basic Life Support certification are required.
