Maintains patient chart folders and retrieves medical records upon request from clinical and non-clinical requestors.
- To receive medical records and to arrange and file them in the appropriate location.
- To assemble and process records as per recommended procedures.
- Identifies errors in the master patient index (MPI) and initiates corrective measures according to established procedures.
- Assist the Nurses with data entry job of patient’s records.
- Keeps health care providers informed by communicating availability or unavailability of the record.
- Keeps the head of department informed of problems or issues; monitors supplies needed; performs other duties as assigned.
- To be capable of working in all units of medical records and to carry out the work of that unit as per recommended procedures.
- Prepare files for the scanning process.
- Understand and follow department’s organizational policies and procedures.
- To maintain confidentiality of patient information and prevent disclosure to unauthorized persons.
- To release information to the patient/authorized persons as per the requests
- Perform periodic inspections of materials or files to ensure correct placement, legibility, and proper condition.
- Exercise good judgment and maintain confidentiality in maintaining critical and sensitive information, records, and reports.
- Attends Coding workshops as conducted by DOH and Insurance companies.
- Performs any other job assigned by the reporting Supervisor/Manager

- Any Diploma or High School Certificate
- AAPC (American Academy of Professional Coders) preferred
