Care Coordinator

_Our company is looking for a Care Coordinator to join our team._

**Responsibilities**:

- Other duties as assigned;
- Performs other duties as required;
- Serving as a member of the Behavioral Assessment Team;
- Reassessing for continuation of Health Home Services;
- Manage a caseload of lower acuity patients and support those patients in working toward their healthcare goals;
- Develop and lead activities during the After-Camp program;
- Assists with front desk and answering phones;
- Time management, attention to detail and organization;
- Forecast staffing needs and bring to the attention of the Administrator and Nursing Supervisor;
- Work collaboratively with Behavioral Health Medical Practitioner, Registered Nurse, and BHT;
- Maintain notes related to personnel problems and or situations that arise and bring to Administrator and Nursing Supervisors attention;
- Informs staff and patients of schedule changes immediately.

**Requirements**:

- The individual in this role must exercise proper phone etiquette and have the ability to navigate proficiently through computer software systems;
- Must be team-oriented and work extremely well with patients, colleagues, physicians and other personnel in a professional and courteous manner;
- Must meet the same training and competency requirements of the aides being supervised;
- Must be honest, ethical, fair, dependable, respect confidentiality and the rights and privacy of others;
- Must meet all health requirements;
- Must have the ability to develop systems for monitoring resident health conditions and for ensuring efficient and safe delivery of resident care;
- Excellent verbal communication and customer service skills;
- Time management, attention to detail and organization;
- Has excellent personal grooming, hygiene and professional demeanor;

**Job Types**: Full-time, Permanent

**Salary**: From AED4,000.00 per month

Ability to commute/relocate:

- Dubai: Reliably commute or planning to relocate before starting work (required)
