**Assessment and Monitoring**: Conducting comprehensive assessments of the patient's health, including vital signs, symptoms, and overall condition. Regularly monitoring the patient's progress, observing any changes or complications, and promptly reporting concerns to the healthcare team.

**Medication Management**: Ensuring patients take their prescribed medications correctly and on time. Monitoring medication effectiveness and potential side effects. Educating patients and their families about medications and their proper administration.

**Wound Care and Dressing Changes**: Managing and dressing wounds, incisions, or injuries as required. Following proper aseptic techniques to prevent infections and promote healing.

**Medical Procedures**: Performing various medical procedures as needed, such as administering injections, catheter care, tracheostomy care, and other specialized treatments.

**Health Education**: Educating patients and their families on managing their health conditions, promoting healthy habits, and preventing complications.

**Assistance with Daily Activities**: Helping patients with activities of daily living (ADLs) such as bathing, grooming, dressing, eating, and mobility, especially if the patient has limited physical abilities.

**Nutritional Support**: Assisting patients with dietary needs, ensuring they receive appropriate nutrition and hydration.

**Pain Management**: Monitoring and managing pain levels, and administering pain relief measures as prescribed.

**Emotional Support**: Providing emotional support to patients and their families during difficult times, fostering a positive and caring environment.

**Medical Equipment Management**: Ensuring that any medical equipment used at home, such as ventilators, oxygen tanks, infusion pumps, etc., is set up correctly, properly functioning, and utilized safely.

**Communication and Collaboration**: Maintaining effective communication with the patient's primary healthcare team, including doctors, therapists, and other specialists, to coordinate care and implement appropriate treatment plans.

**Record-Keeping**: Maintaining accurate and up-to-date patient records, including health assessments, treatments provided, and any changes in the patient's condition.

**Emergency Preparedness**: Being prepared to handle medical emergencies, providing immediate care and coordinating with emergency services if necessary.

**Infection Control**: Following strict infection control protocols to minimize the risk of spreading infections to vulnerable patients.

**Advocacy**: Being an advocate for the patient, ensuring their needs and preferences are considered and respected in the care process.
