**BASIC SKILLS REQUIREMENTS**:
Home Nursing Care

Trained in Data collection & validation

Literacy in English/Arabic speaking abilities

Oriented on KPI required by regulatory body

**General responsibilities of the position include**:

- Ensure health and welfare needs of patients are met
- Provide supervisory and support role for care staff, deploying team appropriately and ensuring a high level of care is offered to patients daily
- Maintain an awareness of all patient’s needs, providing counselling, advice, and practical assistance as appropriate
- Promote and secure the implementation of best practice in the prevention and control of infection
- Observance of Health and Safety requirements
- Report possible hazards to Home Manager and take immediate action where relevant
- Liaison with colleagues and other staff
- Report requirements for equipment and stores
- Liaise with relatives and other health professionals
- Maintain requirements of professional qualifications
- Attend training sessions to extend personal knowledge in relation to nursing care and procedures
- Undertake training and supervision of other staff
- Keep the Nurse Supervisor informed

**Day to day duties**:

- Receive handover and feedback from previous shift.
- Review handover report and deploy staff accordingly
- Tour home and observe each patient
- Check diary for day’s events
- Feedback to staff on duty and cascade information
- Ensure telephone is answered in timely manner
- Monitor personal care requirements and presentation of patient
- Monitor “unacceptable practices”
- Respond to staff feedback and problem areas
- Monitor daily reports by care staff and take appropriate action where required
- Perform comprehensive nursing assessment on patient’s registration under SHG Home Healthcare
- Develop nursing care plan based on physician’s orders and nursing assessment
- Review and amend care plans and update risk assessments
- Administer medications as prescribed by the physician
- Check notes to ensure all events are acted upon and to record any follow up as required
- Ensure to endorse patient status to next shift
- Check drug storage and audit
- Prepare MAR sheets

**Supports the KPI facilitator to report the KPI as per defined time frame**
- Knows and understands the KPI Profile in detail
- Is familiar with the core process
- Able to generate raw data
- Collects valid data as per approved definition
- Communicates with KPI Facilitator the challenges in collecting valid data
- Submit the collected data to KPI facilitator on defined timeframe
- Participates in trainings for data collectors

**Specific JAWDA Roles & Responsibilities**
- Should adhere to the approved structure of data collection process of KPI
- Verify all the data prior submitting to KPI Facilitator from approved resources
- Participates in Internal Data Validation Audit (if required)
- Should attend meetings and trainings related to process improvement and KPI discussions if any

**Medical Records Coordinator**
- File all patients’ medical records and information
- Supply the nursing department with the appropriate documents and forms
- Make sure all medical records are protected and kept confidential
- Gather Patients demographic and personal information
- Help with departmental audits and investigation
- Maintain quality and accurate records by following homecare documentation procedures
