We are currently seeking a highly skilled and compassionate Supervisor Nurse to join our dedicated team in the home health care industry. As a Supervisor Nurse, you will play a crucial role in overseeing and coordinating the delivery of exceptional care to our patients in their homes. Your primary responsibility will be to ensure that all nursing activities are carried out efficiently, effectively, and in compliance with regulatory standards.

**Responsibilities**:
Provide leadership and supervision to a team of nurses and home health aides.

Develop and implement care plans, ensuring that they are individualized and based on patient needs and preferences.

Conduct comprehensive assessments of patients' health status, including physical, psychological, and social factors.

Coordinate and schedule home visits for patients, ensuring adequate coverage and timely delivery of care.

Collaborate with physicians, therapists, and other healthcare professionals to develop and modify patient care plans as necessary.

Monitor and evaluate the quality and effectiveness of care provided by the team, identifying areas for improvement and implementing corrective actions.

Provide clinical guidance and support to the nursing staff, assisting with complex patient cases and addressing any concerns or issues.

Ensure compliance with all relevant regulations, policies, and procedures in home health care.

Participate in the recruitment, orientation, and ongoing training of nursing staff.

Maintain accurate and up-to-date documentation of patient care activities, including assessments, interventions, and outcomes.

Qualifications:
Current registered nurse (RN) license in good standing.

Minimum of [5] years of nursing experience, preferably in home health care or a related field.

Proven experience in a supervisory or leadership role, with the ability to motivate and guide a team effectively.

Strong clinical assessment and critical thinking skills.

Excellent communication and interpersonal skills, with the ability to interact with patients, families, and healthcare professionals.

In-depth knowledge of home health care regulations, policies, and best practices.

Proficiency in using electronic medical record (EMR) systems and other relevant software.

Ability to prioritize tasks, manage time effectively, and work independently.

Flexibility to adapt to changing needs and situations.

CPR certification (BLS for healthcare providers).
