POSITION:RN Field Nurse

ORGANIZATION:Jordan Health Services Medicare

REPORTSTO: RNClinical Team Manager or Administrator

POSITIONSUMMARY:

The RN Field Nurse is responsible forthe timely performance, supervision, training, and documentation ofclinical service delivery to the patient and his/her caregiver inaccordance with accepted standards of nursing practice andregulatory guidelines. S/he assists the RN Clinical Team Manager inimproving the clinical and financial performance of thelocation.

DUTIES,RESPONSIBILITIES & COMPONENTPROPORTIONS:

I. Work FlowNecessary to Ensure Clinical & Financial Success(component proportion 75%)

  • Reports availability routinely,preferably weekly, to the scheduler.
  • Embracesand adheres to the use of electronic documentation via theorganization s software solution.
  • Completesassigned Work Flow tasks within timeframes allotted by theDivision.
  • Responds timely to patientassignments and also responds in accordance with Divisionalguidelines when s/he will not be able to acceptassignments.
  • Performs and documents nursingservice delivery related to patient admission and recertificationincluding the OASIS data set and Plan ofCare.
  • Completes OASIS data set information, asassigned, regarding patient events, i.e. Transfer to In-PatientFacility, Death In Home, Resumption of Care, Discharge, andFollow-Up.
  • Obtains and documents physicianorders, when appropriate.
  • Responsible for thesubmission of all documentation and receipt of physician orders toensure timely locking of OASIS and timely submission ofbilling.
  • Submits nursing orders anddocumentation, when appropriate.
  • Reviews dataregarding patient assignments including but not limited to OAISS,Plan of Care, and Coordination Notes prior to servicedelivery.
  • Submits request for new supplyrequisitions when a new supply order isneeded.
  • Responds to requests from locationstaff to clarify information needed for the Plan of Care (485) orOASIS.
  • Documents OASIS changes in theCoordination notes, when applicable.
  • Submitsrequest to Add New Facility Event, asappropriate.
  • Assists with intake of patientreferrals and in coordination of services between disciplines andother healthcare providers.
  • Participates in thereporting of lab results and/or abnormal findings to theappropriate healthcare professional, physician, and/or fieldclinician.
  • Provides supervision of home healthaides and LVN/LPN team members, asassigned.
  • Participates in patient caseconferences and documents ongoing Coordination of Care with othermembers of the healthcare team.

II. QAPI Activities/HomeHealth Compare Reports (component proportion10%)

  • Reports anddocuments Client Complaint and Client Occurrence Events, whenappropriate.
  • Participates in the QAPI processof the agency, seeking to identify opportunities for performanceimprovement.
  • Reports significant patientfindings to the RN Case Manager and the RN Clinical Team Managerand collaborates on actions or interventions to betaken.
  • Reports and documents patient infectionsper policy.

III. Program Administration Coordination(component proportion 15%)

  • Responsible for the appropriate useof agency resources including, but not limited to, supplies andpersonnel to ensure financial success.
  • Shareson-call responsibilities, as scheduled.
  • Worksto maintain positive, collaborative relationships with patients,caregivers, referral sources, and/orphysicians.
  • Maintains availability at all timesduring his/her scheduled hours of work, either in person or bytelecommunications.
  • Maintains current knowledgeof nursing practice and is responsible for personal continuingeducation required by the BNE.
  • Participates ininternal staff meetings and training activities, asassigned.
  • Collaborates with members of themarketing team to assist in identifying potential referral sources,and patients appropriate for homecare.
  • Performs all other duties asassigned.

JOBREQUIREMENTS:

Education

In accordance with licensingregulations, must meet the following criteria:

Must be a Registered Nurse (RN) licensed in the state ofpractice or in accordance with the Board of Nurse Examiners rulesfor Nurse Licensure Compact (NLC).

  • Experience
  • Minimum of two (2) years of active experienceas a RN, preferred.
  • Possession of a workingknowledge of Medicare/Medicaid home health care benefits, policiesand procedures preferred.
  • Understanding ofMedical Diagnosis Coding, preferred.
  • Skills& Abilities
  • Has a positive attitudeand willingness to learn throughoutemployment.
  • Self-starter with excellent verbaland written communication skills.
  • Ability towork with both clinical and non-clinical staffmembers.
  • Willingness to learn and utilizesoftware solution, inclusive of Divisional Work FlowTasks.
  • Must be organized and have the abilityto manage multiple tasks & priorities at any giventime.
  • Possesses a firm foundation of basicnursing skills.
  • Willingness to learn homehealth regulatory standards related to job-specific duties,including but not limited to: OASIS, Condition of Participation,state-specific licensing rules, diagnosis coding, and internalpolicies.
