Tches et responsabilit s:

  • Recueillirtoutes les donn es jug es pertinentes concernant les caractristiques et les besoins desusagers
  • Procder l valuation des usagers en utilisant les techniques appropri eset r diger les rapports ncessaires
  • Agirtitre d intervenant direct ou indirect en consultation et enorientation desusagers
  • Recommanderl orientation g n rale du plan d intervention adapt aux besoins desusagers en prenant en consid ration les ressources de ltablissement
  • Contribuerfournir un support clinique et assurer une fonction conseil aupr sde sespairs;
  • Agirtitre de personne-ressource et de support professionnel aupr s despairs rencontrant des difficult s particuli res lors de ltablissement du plan d intervention ou lors de la r vision du plandintervention
  • valuer, selon les m thodes tablies, les r sultats atteints par leplan d intervention et y recommander les ajustements n cessaires, sil y alieu
  • laborer et animer des programmes d activit s sp cifiques auxbesoins rencontr s au niveau de la clientle
  • Agircomme personne-ressource lors de consultation et de recherche li essaprofession
  • Participeraux discussions de cas dans l tablissement au besoin et selon lesorientations sp cifiques de certainsclients
  • Dvelopper et maintenir les liens de collaboration avec les autresorganismes du minist re de la Sant et des Services sociaux, lesgroupes b n voles ou autres organismes communautaires int ress s laproblmatique
  • Participerdes rencontres avec le chef de service, l quipe etlorganisation

Exigences:

  • Dtenir un baccalaur at en sciences humaines ou science sociales telque criminologie, travail social oupsychologie
  • Maitrisede la langue fran aise (anglais unatout)
  • Membredun ordreprofessionnel
  • Dtenir un permis de conduirevalide
  • Possder un vhicule
