KeyResponsibilities:
- ElderlyCare:Provide compassionate inhome nursing care to elderly clientsincluding medication administration monitoring vital signs andmanaging chronic health conditions. Assist with daily activitiespersonal hygiene mobility support and meal preparation. Develop andupdate individualized care plans with input from clients familiesand healthcare professionals responding promptly to changes inhealthstatus.
- Patientand FamilyEducation:Provide essential health education to patients and familiesfocusing on effective condition management and medical equipmentuse.
- CoordinationandCommunication:Work closely with physicians and other healthcare professionals toensure continuity of care and accurate patient statusupdates.
- DocumentationandCompliance:Maintain precise and uptodate patient records adhering to safetyprotocols and infection control procedures ensuring that all carepractices align with DHA/DOHregulations.
- TrainingandDevelopment:Design and implement training programs for new and existingcaregiving staff covering best practices DHA compliance and safetyprotocols. Lead workshops handson demonstrations and evaluatetraining effectiveness providing ongoing support and mentorship tonurture a culture ofexcellence.
- DocumentationandCompliance:Maintain accurate client records including care plans and incidentdocumentation. Ensure compliance with DHA standards and homecarepolicies participating in regular audits and reviews to upholdquality and safetystandards.
- EducationandCertification:Bachelors degree in Nursing or equivalent valid DHA/DOH license andgeriatric nursing certificationpreferred.
- Experience:Minimum 3 years in elderly care ideally in a homecare setting;experience in training and team development is highlydesirable.
- SkillsandCompetencies:Strong clinical expertise in elderly care effective communicationmentoring skills ability to adapt in a dynamic home careenvironment and proficiency in EMRsystems.
- Outstandinginterpersonal and communication skills with a compassionateapproach to patientcare.
- Flexibilityto work independently and adjust to the dynamic nature of homecareenvironments.
