Job Description

Job Duties

  • Completes an initial assessment of patient andfamily to determine home care Provides a complete physicalassessment and history of current and previous illness(es).

  • Regularly re-evaluatespatient nursing

  • Initiates the plan of care and makes necessary revisionsas patient status and needs ;

  • Uses health assessment data to determine nursing;

  • Develops a care plan,which establishes goals based on nursing diagnosis and incorporatestherapeutic, preventive, and rehabilitative nursing Includes thepatient and the family in the planning ;

  • Initiates appropriate preventive andrehabilitative nursing Administers medications and treatments asprescribed by the ;

  • Counsels the patient and family in meeting nursing andrelated

  • Provides healthcare instructions to the patient as appropriate per assessment andplan of ;

  • Identifiesdischarge planning needs as part of the care plan development andimplements prior to discharge of the ;

  • Make written and oral reports/recommendationsto the DON concerning the operation of the nursing unit/shift

  • Assure that the unitarea is maintained in a clean and safe manner for patient comfortand convenience by assuring that necessary equipment and suppliesare maintained to perform such duties/services
